Mr. Li, male, 74 years old, first consultation on December 21, 1987.
On November 11, 1987, due to repeated frequent urination, thinning of the urine stream, and gross hematuria for 10 years, which had been worsening day by day for 20 days, the surgical diagnosis was benign prostatic hyperplasia. He underwent surgical treatment on December 2. He was discharged from the hospital on December 12 of the same year. The patient had a previous history of coronary heart disease.
On December 21, because of urinary incontinence for more than two weeks after the surgery, he came to our hospital for further diagnosis and treatment again. The patient had no symptoms of urethral pain. He only felt dry mouth at night, had poor sleep, occasional palpitations, normal stools, and the urine test showed no abnormalities. The pulse was irregular. The diagnosis in traditional Chinese medicine was urinary incontinence. This was caused by deficiency of both qi and yin after the surgery, and the failure of the kidney to control urination. The treatment principle was to tonify qi and yin, strengthen the kidney to reduce urination, and assist with promoting blood circulation and calming the heart. The modified Yiqi Huoxue Decoction (a self-formulated decoction for tonifying qi and promoting blood circulation) was used:
15 grams of Codonopsis pilosula, 15 grams of Salvia miltiorrhiza, 10 grams of Angelica sinensis, 10 grams of Radix Paeoniae Rubra, 6 grams of Ligusticum chuanxiong, 10 grams of Mantidis Ootheca, 10 grams of Alpinia oxyphylla, 15 grams of Rosa laevigata, 10 grams of Rubus chingii, 10 grams of Cuscuta chinensis, 10 grams of Semen Platycladi, 15 grams of Ophiopogon japonicus. One dose per day, decocted in water for oral administration.
Second consultation on December 24: After taking 3 doses of the medicine, the urinary incontinence decreased, the dry mouth was relieved, the sleep was better, and there was no palpitations. The tongue coating was thin and white, and the pulse became regular. In view of the disappearance of the irregular pulse, Salvia miltiorrhiza, Radix Paeoniae Rubra, and Ligusticum chuanxiong were removed from the previous prescription, and 6 grams of Schisandra chinensis, 6 grams of Psoralea corylifolia, and 12 grams of Euryale ferox were added to strengthen the function of tonifying the kidney and astringing. After taking the medicine for two weeks, the urinary incontinence was cured. Up to now, more than a year has passed, and there has been no recurrence.
